Ayushman Bharat Card 2026 — How to Apply Online, Check Eligibility & Find Hospitals

Method 1: Official PMJAY Portal

1. Visit beneficiary.nha.gov.in
2. Enter your mobile number → receive OTP
3. Select your state
4. Search by name, Aadhaar, ration card, or mobile number
5. If eligible, your name and family details will appear

Method 2: Ayushman Bharat Helpline

Call 14555 (toll-free) and provide your Aadhaar or ration card number. The operator will check your eligibility immediately.

Method 3: Visit CSC or Empanelled Hospital

Any CSC center or PM-JAY empanelled hospital can check your eligibility using biometric verification.

Step 1: Download ABHA App

1. Download "Ayushman Bharat Health Account (ABHA)" app from Google Play Store or App Store
2. Open the app and tap "Create ABHA"

Step 2: Create Your ABHA Health ID

1. Choose verification method: Aadhaar (recommended) or Driving License
2. Enter your Aadhaar number
3. Verify with OTP sent to Aadhaar-linked mobile
4. Fill in your details — name, age, address
5. Your ABHA Health ID is created!

Step 3: Get Your Ayushman Bharat Golden Card

1. Visit your nearest CSC center or empanelled hospital
2. Carry: Aadhaar card, ration card, mobile phone
3. The operator will verify your SECC database eligibility
4. Biometric authentication (fingerprint/iris)
5. Your Golden Card is printed on the spot — free of cost!

Cost: Everything is free. No premium, no fees, no charges.

1. Visit hospitals.pmjay.gov.in
2. Select your state → district → hospital type (government/private/both)
3. Filter by specialty if needed (cardiology, orthopedics, oncology, etc.)
4. View the complete list with hospital name, address, and contact details

What to Do When You Need Treatment

1. Go to any PM-JAY empanelled hospital (government or private)
2. Show your Ayushman Bharat card or Aadhaar at the Ayushman desk
3. The hospital verifies your eligibility and pre-approves treatment
4. Get treated — 100% cashless, no money needed
5. The hospital claims the cost directly from the government

Coverage: Up to ₹5 lakh per family per year for 1,900+ medical procedures including heart surgery, knee replacement, cancer treatment, and more.

Problem	Solution
"Name not found in SECC data"	Your family may not be in the 2011 census database. Contact your district PMO or call 14555 for appeal.
"Aadhaar OTP not received"	Ensure mobile number is linked to Aadhaar. Visit UIDAI center to update mobile if needed.
"Already have a card but it's not working"	Visit CSC center for re-verification. Old cards may need renewal with updated biometrics.
"Hospital refusing Ayushman card"	Confirm the hospital is empanelled on hospitals.pmjay.gov.in. If empanelled and refusing, call 14555 to report.
"Treatment not covered"	Check the list of 1,900+ procedures at pmjay.gov.in. Some cosmetic and fertility treatments are excluded.
